OUR HISTORY    

  • Founded in 2010 by Dr. Vigen Vick Abovian MD, our organization is committed to delivering healthcare to underserved patients with diverse needs, addressing chronic problems, urgent issues, and convenient care.

  • Our experienced providers utilize extended team-based care to improve patient access and healthcare outcomes.



OUR TEAM CARE    

  • Our team-based care, supported by our providers, involves an integrated operational infrastructure with case management and care gap coordinators to effectively address the healthcare needs of a large volume of patients.

  • Language Fluency:

    • By Staff: English, Armenian, Russian.

    • By Provider: English, Armenian, Spanish, Russian.



LA COUNTY SPA2    

  • Glendale and surrounding areas are located within Service Planning Area (SPA2).

  • About one-third to nearly half residents within SPA2 communities, live at or under twice the Federal Poverty Level (FPL).

  • Medi-Cal Expansion In California will extend full-scope Medi-Cal to all low-income adults, REGARDLESS OF IMMIGRATION STATUS, Beginning January 1, 2024. We are proud to be the primary care provider of choice for those in need.

REFERRALS    

  • We deliver services to patients above 21 years of age designated by Health Plans for adult primary care.

  • Referrals to Pediatrics, Vision, Dental Care, and other Specialists are coordinated through in-network contracted providers.



PATIENT ACCESS    

  • Please call our office first, and present your concern to our Team Care:

    • your issue may he handled remotely through our Case management,

    • or by scheduling an in-person office visit for evaluation.

  • If you need After-Hours care, or your issue is still not resolved:

    • leave a mesage for provider on call at 818-548-8001, Ext #1.

    • You will get a callback within 30 minutes.

  • Urgent needs: are addressed on the Same Day within 8 hours.

  • Convenient care and Chonic problems: are addressed within 48 hours.

  • Our patients can view their records online on PatientAlly, after activating their account with our staff.
